HOW TO REQUEST YOUR MEDICAL RECORDS?
If you would like to obtain a copy of your medical records, please print and complete a Request form. We will respond to your request for medical records within 30 days of receipt. If you require a notarized copy of your medical/billing records, PEAK Sport and Spine charges a fee for the preparation of your medical records in compliance with the State of Missouri laws.
Completed forms may be submitted in the following methods:
- Presented to any PEAK Sport and Spine facility
